A 69-year-old Italian woman reported a history of chronic atrial fibrillation, a history of non-insulin dependent diabetes, a history of combined biological mitral prosthesis and coronary artery bypass grafting with left internal mammary artery graft to the left anterior descending artery surgery intervention occurred a month ago requiring also a pacemaker implantation with following fever and discomfort. Echocardiographic evaluation revealed mitral prosthesis endocarditis vegetations, a left atrial dilatation and a suspected mitral prosthesis leak (Fig. 1). A mild–moderate tricuspid insufficiency was also seen. Acinetobacter baumannii hemoculture positive was found. Intensive care units are areas where resistance problems, including main resistance phenotypes, constitute a major problem for the intensivist's clinical practice. Also this case focuses attention on rare and serious A. baumannii infective endocarditis and on difficulties of intensivist's clinical practice.
