This paper is based on one presented to the Research Seminar in Psychoanalytic Medicine at the Columbia University Psychoanalytic Clinic for Training and Research in March, 1962. Many colleagues have contributed to the work reported on in this paper, which was done under the auspices of the Community Mental Health Program of the Harvard School of Public Health under the direction of Dr. Gerald Caplan. I wish particularly to thank Robert Rapoport for his help at all stages of this work. In addition, thanks are due to the following for help at different stages of the research: Thomas Brigante, Ivor Browne, Peggy Golde, Beatrice Horvitz, David Kaplan, Thomas Plaut and Rae Sherwood.
