This paper is based on one presented to the Research Seminar in Psychoanalytic Medicine at the Columbia University Psychoanalytic Clinic for Training and Research in March, 1962. Many colleagues have contributed to the work reported on in this paper, which was done under the auspices of the Community Mental Health Program of the Harvard School of Public Health under the direction of Dr. Gerald Caplan.
