According to the 1998 annual report published by UNOS (United Network for Organ Sharing), more than 16,000 transplants from cadaveric donors and 3700 living related transplants were performed in 1987. Thus, the number of anesthesiologists involved in transplantation surgery is ever increasing, not only in providing anesthesia for the transplantation itself, but also taking care of patients with previous transplantation who present for other operations. Consequently, there is a need for a textbook of anesthesia and transplantation. The book starts with a history of the science of transplantation, and gives a good synopsis of the basis of immunosuppression. There are chapters on the pathophysiology of end organ failure and management and special requirements of specific organ transplants. The discussion of end stage renal disease and its implications in anesthetic management is excellent. Another chapter discusses the management and anesthetic implications of patients who have had a transplant and return for another procedure. Every aspect of the transplantation process is discussed, from donor and recipient selection and intraoperative management to postoperative complications and psychosocial issues involved in transplantation. Living donors are now widely used for renal transplantation and becoming more accepted for partial liver transplantations. The number of living donors has doubled since 1988 (UNOS 1998 annual report). Management of these patients is not discussed in this book, despite recent interest in efforts to reduce length of stay and postoperative morbidity in donors. In summary, this book is a comprehensive text, well written with numerous excellent references covering almost every aspect of transplantation and the anesthetic implications and should have wide appeal to both anesthesia residents and established practitioners involved in the care of these patients. It is an excellent book and highly recommended as a good addition to one’s personal library.
