ResumoWe report a case of right ventricular perforation by a passive-tined permanent pacemaker lead detected 1 month after implantation. The pacemaker lead was seen to perforate through the myocardium and pericardium and moving freely in the mediastinum with no evidence of pericardial effusion. Extraction was uneventful.
