ResumoTwo siblings with nephrogenic diabetes insipidus, in whom progressive dilation of the upper urinary tract was demonstrated, are reported. In one child this was accompanied by focal pyelonephritis with glomerular and tubular atrophy. Urologic evaluations and radiographic studies failed to reveal an organic obstructive etiology; it is hypothesized that these changes were caused by a "functional obstruction" due to the large urine volume in a distensible urinary tract. The necessity of serial urologic evaluations in all such patients is stressed.
