A 46-year-old man was admitted with weakness and episodes of presyncope. He had experienced palpitations for 12 years, during which time he had taken oxprenolol. More recently, a period of atrial fibrillation had been documented, an exercise test and echocardiogram were normal, and he had been prescribed sotalol. There was no relevant past medical history; in particular, neither he nor any relatives had ever suffered a spontaneous thrombosis. The initial electrocardiogram (ECG) showed sinus
