Learn more.Copy URL Share a linkShare onEmailFacebookTwitterLinkedInRedditWechat Summary Preliminary investigation of acidification and concentration power of each kidney in two patients with predominantly unilateral hydronephrosis due to congenital ureteropelvic obstruction, the following conclusions may be drawn. In the hydronephrotic kidney there was an impairment of the water reabsorption and a defective acid-base regulation. As maximal reabsorption of bicarbonate took place in the hydronephrotic kidney during an acid load, the proximal parts of the tubules cannot have been severely damaged. Signs of distal tubular damage were suggested by a limited capability of the kidney to lower the pH of the urine, and to produce a concentrated urine. En se basant sur ľ'investigation préliminaire du pouvoir ? acidification et de concentration de chaque rein chez 2 malades avec une hydronéphrose principalement unilatérale, due à une obstruction ultéro-pelvienne congénitale, on peut tirer les conclusions suivantes: Dans le rein hydronéphrotique, il y avait une diminution dans la réadsorption ? eau et une régulation defectueuse de la base d'acide. Etant donné qu'une réadsorption maxima de bicarbonate s'observe dans le rein néphrotique pendant une charge ?acide, les parties voisines des tubules ne peuvent pas être sérieusement lésées. Auf Grund einer vorläufigen Untersuchung betreffend Säuerung und Koncentrationsvermögen der beiden Nieren zweier Patienten mit vorwiegend unilateraler Hydronephrosis, verursacht durch congenitalen Ureter -Nierenbecken-Verschluss, können folgende Sehlussfolgerungen gezogen werden: In der hydronephrotischen Niere bestand eine verschlechterte Wasserrückresorp-tion und eine defekte Saure-Basen-Regulation. Eine maximale Rückresorption von Bikarbonat fand während einer Säurebelastung in der hydronephrotischen Niere statt, die proximalen Teile der Tubuli können also nicht ernstlich beschädigt worden sein. Ein Schaden an den distalen, tubulären Teilen wird vermutet, da die Niere nur eine begrenzte Fähigkeit zur Herabsetzung des pH des Urins sowie zur Produktion eines konzentrierten Urins aufwies. Estudios de la función renal por separado en hidronefrosis predominantemente unilaterales. Del estudio preliminar de la capacidad de acidificación y concentración de cada riñón, en dos pacientes con hidronefrosis predominantemente unilateral por obstructión congénita ureteropélvica, se desprenden las siguientes conclusiones. En el riñón hidronefrótico existía tin transtomo de la reabsorción hídrica y una regulateón acido-base deficitaria. Las porciones proximales de los tubulos no podían estar gravemente lesionados ya que tuvo lugar una reabsorción de bicarbonato máxima, en el riñón hidronefrótico, durante la sobrecarga ácida. La limitatión de la capacidad del riñón para descender el pH de la orina, y segregar una orina concentrada sugerían la existencia de lesiones tubulares distales. References Bowman, R.
