Gonorrhea in the median raphe of the penis is seldom reported. Bernfeld in a recent article presents a case report and a review of the world literature on gonococcal infections and structural anomalies of the median penoscrotal raphe, with an extensive bibliography. Report of Case A 25-year-old Caucasian soldier was seen because of a somewhat tender swelling on the ventral aspect of the penis, with discharge of pus, of four days' duration, onset two days following sexual intercourse. There was no urethritis. Examination revealed a small, poorly outlined, tender, red swelling in the median raphe of the penis near its middle. Creamy pus could be expressed from a broad funnel-shaped opening into a blind pouch in the area of the swelling, smears of which revealed gram-negative intracellular diplococci, morphologically typical of Neisseria gonorrheae. There had never been any previous inflammation or discharge, nor any passage of urine from this area.
