ResumoA 74-year-old man was noted to have pupils that did not react to light but did constrict with accommodation (Figure 1). His neurologic examination results were otherwise normal. Serum was positive for rapid plasma reagin at 1:32, and cerebrospinal fluid was positive for Venereal Disease Research Laboratory testing at 1:2; serum human immunodeficiency virus antibody testing was negative. The patient was treated with intravenous penicillin for neurosyphilis.
