Comparison between bioartificial and artificial liver for the treatment of acute liver failure in pigs

AIM:To characterize and e�a��uate the thera�eutic e��icacy o� bioarti�icia�� ��i�er (B�L) as com�ared to that o� continuous hemodia�i��tration (�HDF) with ���asma e�change (PE), which is the current standard thera�y �or �u��minant he�atic �ai��ure (FHF) in Ja�an. METHODS:Pigs with he�atic de�ascu��arization were di�ided into three grou�s: (1) a non-treatment grou� (NT�� n = 4)�� (2) a B�L treatment grou� (B�L�� n = 4),(3) a PE + �HDF treatment grou� using 1.5 L o� norma�� �orcine ���asma with �HDF (PE + �HDF, n = 4).Our B�L system consisted o� a ho����ow �iber modu��e with 0.2 μm �ores and 1 � 10 10 o� microcarrier-attached he�atocytes inocu��ated into the e�tra-�iber s�ace.Each treatment was initiated 4 h a�ter he�atic de�ascu��arization. RESULTS:The �igs in the B�L and the PE + �HDF grou�s sur�i�ed ��onger than those in the NT grou�.The e��imination ca�acity o� b��ood ammonia by both B�L and PE + �HDF was signi�icant��y higher than that in NT. �romatic amino acids (���) were se��ecti�e��y e��iminated by B�L, whereas both ��� and branched chain amino acids, which are bene�icia�� �or ��i�e, were e��iminated by PE + �HDF.E��ectro��ytes maintenance and acid-base ba��ance were better in the �PE + �HDF grou� than that in the B�L grou�. CONCLUSION:Our resu��ts suggest that PE + �HDF e��iminate a���� �actors regard��ess o� bene�its, whereas B�L se��ecti�e��y metabo��izes to�ic �actors such as ���.Howe�er since PE + �HDF maintain e��ectro��ytes and acid-base ba��ance, a combination thera�y o� B�L ���us �PE + �HDF might be more e��ecti�e �or FHF.
