Right-sided Hemothorax and Recurrent Abdominal Pain in a 34-Year-Old Woman

ResumoA 34-year-old woman presented with a 1-week history of worsening abdominal pain and dyspnea. During the previous several years, the patient had experienced similar bouts of recurrent abdominal swelling and pain that had more recently become associated with dyspnea. Previous emergency room evaluations had detected a right-sided pleural effusion. Thoracenteses performed on several occasions relieved the dyspnea after removal of grossly bloody pleural fluid. Pleural fluid cytologic studies and pleural biopsies had been unrevealing. A ventilation/perfusion lung scan performed during one of the episodes was normal. The patient had otherwise been in good health except for a history of menorrhagia and a miscarriage 9 years previously.
