ResumoTrochlear dysplasia is a common cause of recurrent patellar instability. It is most frequently seen in adolescent female patients with a history of patellar dislocations. Dysplasia is generally classified using the 4-part Dejour classification system. In certain patients, a sulcus-deepening trochleoplasty in conjunction with a medial patellofemoral ligament reconstruction or a tibial tubercle osteotomy may permanently restore normal joint stability and lead to improved outcomes after surgery. Although short-term outcomes seem favorable, additional long-term follow-up studies are needed to further define the effectiveness of trochleoplasty for preserving the patellofemoral articular cartilage.
