This chapter discusses the adverse effects of positive inotropic drugs, such as digoxin and milrinone. There is currently no good evidence that the use of digoxin is associated with increased mortality in patients with atrial fibrillation, with or without concurrent congestive heart failure. A proper prospective randomized study would be necessary to confirm or refute this hypothesis. The interaction of digoxin with quinine has only occasionally been described. It seems to have the same basis as the interaction of digoxin with quinidine. Milrinone by inhalation reduces pulmonary artery pressure and its use has been studied retrospectively in high-risk patients. Antidysrhythmic agents of class III have been reviewed, including the newer agents, dofetilide, ambasilide, azimilide, chromanol 293B, dronedarone, ersentilide, ibutilide, sematilide, tedisamil, and trecitilide, as well as amiodarone, bretylium, and sotalol.
