This is a revision of an earlier paper written at the University of Pennsylvania. I benefited greatly from the comments of my colleagues at the University of Chicago while a visitor there in 1975–76, from the comments of my colleagues at Ecole Supéerieure des Sciences Economiques et Commerciales while a visitor there in 1976–77, as well as from the comments of my colleagues at the University of Pennsylvania.
