The nasal base para-alar region has been recognized as an important region for overall facial assessment. 1 Epker B.N. The Skeletal Nasal Base. Autologous bone graft and alloplastic onlay are the mainstays of such treatments. Le Fort I osteotomy for advancement is often believed to be helpful for aesthetic improvement for such para-alar deficiencies. Very few articles have illustrated any practical regional osteotomy techniques that have gained wide popularity. Here we describe a new surgical technique for para-alar augmentation in which the pyriform rims are outfractured anterolaterally and maintained with a free interposition bone graft.
