ResumoPosterior tibial tendon tenosynovitis is now a recognized entity no longer confused with an ankle sprain. This article describes how the anatomy of the tendon may lead to an inherent predisposition to tenosynovitis, and discusses various disease entities that may cause the condition. A discussion of current imaging technique reveals that ultrasound may replace MRI as an inexpensive and accurate method to assist in diagnosis. Nonoperative and operative management is reviewed, with particular attention to appropriate timing of intervention. Through this review, the reader may approach posterior tibial tendon tenosynovitis with an algorithmic confidence, leading to a successful outcome and preventing progression of the disease to adult acquired flatfoot.
