ResumoBackground: The choledochal cysts are congenital dilatations of the biliary tract, they are more frequent in woman; 80% are diagnosed before they are 10 years old. The approach in this entity can be surgical or endoscopic according to the of Todani modifi cation of Alfonso-Lej classifi cation. Case report: We present our experience in a complete laparoscopic management treatment Todani 1C for a choledochal cyst in a 19 year old patient from our Hepatobiliar and Pancreatic Clinic, of the General and Endoscopic Surgery Division at the General Hospital «Dr. Manuel Gea Gonzalez» in Mexico City. Results: The procedure was realized by a totally laparoscopic approach in 390 minutes, the patient was discharged from the hospital 5 days later, after a month a colangioresonance was performed which showed bilis fl owing through the Roux-en-Y anastomosis and no stenosis at the bilioenteric reconstruction, at 3 months from the surgery it has shown no complications. Conclusion: Even if the laparoscopic approach is a technically demanding procedure, in experimented groups is a safe treatment to choledochal cysts.
