This article is based on a variety of small research interventions. These include: the observation of a range of health system managers from the chief executive of the National Health Service in England to practicing clinical directors; visits to a wide variety of health system sites; and a series of seminars over several years conducted with the support of the King Edward's Hospital Fund for London.
