Multi-institutional outcome analysis of resected intrahepatic cholangiocarcinoma in North and South America

Resumo

Introduction: In the last five years there have been major advances in the prognostic stratification of intrahepatic cholangiocarcinoma derived from international collaborative efforts. As such, this study aims to characterize survival outcomes and prognostic features after curative resection of intrahepatic cholangiocarcinoma at four referral centres in North and South America. Methods: Retrospective review of all patients who underwent resection of intrahepatic cholangiocarcioma between 2000 and 2015 at the participating institutions: AC Camargo Cancer Center, Brazil; Foothills Medical Centre, Canada; Hospital General de Agudos Dr. Cosme Argerich, and Sanatorio Allende, in Argentina. Demographics, operative and pathologic data, and survival outcomes were abstracted and analyzed in a standard fashion. Results: A total of 56 patients underwent curative surgery – 22 extended, 19 hemi-liver and 15 minor liver resections (≤ 3 segments). Median tumor size was 7.2 cm (IQR: 4.4–10.0), and multifocal disease was found in 12 (21%) patients. Margin status was positive in 10 (18%) cases. The 3-year disease-free survival was 28.6%. Recurrences occurred more often in the liver (20/31); within two years from surgery (20/31); and treatment was undertaken in 17 cases. Median survival after recurrence was 13 months. Conclusions: Despite aggressive surgical treatment of intrahepatic cholangiocarcinoma, long-term outcomes are still poor.
