However, it is unknown whether the additional tumors detected are medically important. Traditional clinical and pathological features associated with medically important cancer include a palpable tumor, multifocal or diffuse involvement and moderately or poorly differentiated histology. In contrast, microfocal, well differentiated tumors are considered to be possibly medically unimportant. We sought to examine the clinical and pathological tumor stage and tumor grade in 1,169 consecutive men whose prostate cancer was detected during serial PSA based screening protocols involving 24,346 men screened at 6-month intervals. Of the patients 97% had clinically localized (clinical stage T1 or T2) tumors, of which 39% were not palpable (stage T1). Of the men whose cancer was detected through initial screening who underwent surgical staging 69% had pathologically organ confined (pathological stage whose cancer was detected through initial screening pT1 or pT2) disease compared to 74% whose cancer was detected through serial screening (after an initially negative screening). Impalpable, clinically focal, well differentiated minimal tumors were noted in 16% of the men. However, only 3% of the men who underwent surgical staging had impalpable, pathologically microfocal, well differentiated minimal tumors. We conclude that the majority of tumors detected through PSA based screening have the clinical and pathological features associated with medically important prostate cancer. © 1994 by The American Urological Association Education and Research, Inc.FiguresReferencesRelatedDetailsCited ByLoeb S, Roehl K, Helfand B, Kan D and Catalona W (2018) Can Prostate Specific Antigen Velocity Thresholds Decrease Insignificant Prostate Cancer Detection?Journal of Urology, VOL. 183, NO. 1, (112-117), Online publication date: 1-Jan-2010.Loeb S, Kan D, Yu X, Roehl K and Catalona W (2018) Preoperative Prostate Specific Antigen Doubling Time is Not a Useful Predictor of Biochemical Progression After Radical ProstatectomyJournal of Urology, VOL. 183, NO. 5, (1816-1821), Online publication date: 1-May-2010.Yu X, Loeb S, Roehl K, Han M and Catalona W (2018) The Association Between Total Prostate Specific Antigen Concentration and Prostate Specific Antigen VelocityJournal of Urology, VOL. 177, NO. 4, (1298-1302), Online publication date: 1-Apr-2007.Loeb S, Nadler R, Roehl K, Antenor J and Catalona W (2018) Risk of Prostate Cancer for Young Men With a Prostate Specific Antigen Less Than Their Age Specific MedianJournal of Urology, VOL. 177, NO. 5, (1745-1748), Online publication date: 1-May-2007.Graif T, Loeb S, Roehl K, Gashti S, Griffin C, Yu X and Catalona W (2018) Under Diagnosis and Over Diagnosis of Prostate CancerJournal of Urology, VOL. 178, NO. 1, (88-92), Online publication date: 1-Jul-2007.Loeb S, Roehl K, Nadler R, Yu X and Catalona W (2018) Prostate Specific Antigen Velocity in Men With Total Prostate Specific Antigen Less Than 4 ng/mlJournal of Urology, VOL. 178, NO. 6, (2348-2353), Online publication date: 1-Dec-2007.Loeb S, Gonzalez C, Roehl K, Han M, Antenor J, Yap R and Catalona W (2018) Pathological Characteristics of Prostate Cancer Detected Through Prostate Specific Antigen Based ScreeningJournal of Urology, VOL. 175, NO. 3, (902-906), Online publication date: 1-Mar-2006.LOEB S, HAN M, ROEHL K, ANTENOR J and CATALONA W (2018) ACCURACY OF PROSTATE WEIGHT ESTIMATION BY DIGITAL RECTAL EXAMINATION VERSUS TRANSRECTAL ULTRASONOGRAPHYJournal of Urology, VOL. 173, NO. 1, (63-65), Online publication date: 1-Jan-2005.NADLER R, LOEB S, ROEHL K, ANTENOR J, EGGENER S and CATALONA W (2018) USE OF 2.6 NG/ML PROSTATE SPECIFIC ANTIGEN PROMPT FOR BIOPSY IN MEN OLDER THAN 60 YEARSJournal of Urology, VOL. 174, NO. 6, (2154-2157), Online publication date: 1-Dec-2005.RABETS J, JONES J, PATEL A and ZIPPE C (2018) PROSTATE CANCER DETECTION WITH OFFICE BASED SATURATION BIOPSY IN A REPEAT BIOPSY POPULATIONJournal of Urology, VOL. 172, NO. 1, (94-97), Online publication date: 1-Jul-2004.BORBOROGLU P, COMER S, RIFFENBURGH R and AMLING C (2018) EXTENSIVE REPEAT TRANSRECTAL ULTRASOUND GUIDED PROSTATE BIOPSY IN PATIENTS WITH PREVIOUS BENIGN SEXTANT BIOPSIESJournal of Urology, VOL. 163, NO. 1, (158-162), Online publication date: 1-Jan-2000.RIETBERGEN J, HOEDEMAEKER R, BOEKEN KRUGER A, KIRKELS W and SCHRODER F (2018) THE CHANGING PATTERN OF PROSTATE CANCER AT THE TIME OF DIAGNOSIS: CHARACTERISTICS OF SCREEN DETECTED PROSTATE CANCER IN A POPULATION BASED SCREENING STUDYJournal of Urology, VOL. 161, NO. 4, (1192-1198), Online publication date: 1-Apr-1999.RAMOS C, CARVALHAL G, SMITH D, MAGER D and CATALONA W (2018) CLINICAL AND PATHOLOGICAL CHARACTERISTICS, AND RECURRENCE RATES OF STAGE T1C VERSUS T2A OR T2B PROSTATE CANCERJournal of Urology, VOL. 161, NO. 5, (1525-1529), Online publication date: 1-May-1999.ETZIONI R, CHA R and COWEN M (2018) SERIAL PROSTATE SPECIFIC ANTIGEN SCREENING FOR PROSTATE CANCER: A COMPUTER MODEL EVALUATES COMPETING STRATEGIESJournal of Urology, VOL. 162, NO. 3 Part 1, (741-748), Online publication date: 1-Sep-1999.CATALONA W and SMITH D (2018) CANCER RECURRENCE AND SURVIVAL RATES AFTER ANATOMIC RADICAL RETROPUBIC PROSTATECTOMY FOR PROSTATE CANCERJournal of Urology, VOL. 160, NO. 6 Part 2, (2428-2434), Online publication date: 1-Dec-1998.LODDING P, AUS G, BERGDAHL S, FROSING R, LILJA H, PIHL C and HUGOSSON J (2018) CHARACTERISTICS OF SCREENING DETECTED PROSTATE CANCER IN MEN 50 TO 66 YEARS OLD WITH 3 TO 4 NG./ML.
