Indications for the procedure included severe ureteral injury in 4 patients, failed supravesical diversion in 2, renal carcinoma in a solitary kidney in 1, renovascular hypertension in 1 and donor arterial reconstruction before renal transplantation in 29. Success was obtained in all but 2 procedures, both of which involved previously operated kidneys with severe inflammation and adhesions involving the renal pelvis and pedicle. Based on our experience and a review of currently available literature we believe that renal autotransplantation and extracorporeal reconstruction can provide the best solution for patients with severe renovascular and ureteral disease not correctable by conventional operative techniques. The technique can be of particular value in removing centrally located tumors in solitary kidneys and in preparing donor kidneys with abnormal arteries for renal transplantation. The role of autotransplantation in the management of advanced renal trauma and calculus disease is less clear. A long-term comparison of patients treated by extracorporeal nephrolithotomy versus conventional lithotomy techniques will be necessary before a conclusion is reached in these disease categories. © 1977 by The American Urological Association Education and Research, Inc.FiguresReferencesRelatedDetailsCited byKawamura J, Okada Y, Nishibuchi S and Yoshida O (2018) Transient Anuria Following Administration of Angiotensin I-Converting Enzyme Inhibitor (SQ 14225) in a Patient with Renal Artery Stenosis of the Solitary Kidney Successfully Treated with Renal AutotransplantationJournal of Urology, VOL. 127, NO. 1, (111-113), Online publication date: 1-Jan-1982.Sufrin G (2018) Editorial CommentJournal of Urology, VOL. 126, NO. 3, (315-315), Online publication date: 1-Sep-1981.Berg S, Jacobs S, Cohen A, Li F, Marchetto D and Brown R (2018) The Surgical Management of Hereditary Multifocal Renal CarcinomaJournal of Urology, VOL. 126, NO. 3, (313-315), Online publication date: 1-Sep-1981.Palleschi J and McAninch J (2018) Renal Autotransplantation for Retroperitoneal FibrosisJournal of Urology, VOL. 125, NO. 3, (408-411), Online publication date: 1-Mar-1981.Novice A, Stewart B and Straffon R (2018) Extracorporeal Renal Surgery and Autotransplantation: Indications, Techniques and ResultsJournal of Urology, VOL. 123, NO. 6, (806-811), Online publication date: 1-Jun-1980.Lawson R (2018) Extracorporeal Renal SurgeryJournal of Urology, VOL. 123, NO. 3, (301-305), Online publication date: 1-Mar-1980.Novick A, Magnusson M and Braun W (2018) Multiple-Artery Renal Transplantation: Emphasis on Extracorporeal Methods of Donor Arterial ReconstructionJournal of Urology, VOL. 122, NO. 6, (731-735), Online publication date: 1-Dec-1979.
