Complications of Thoracoabdominal Retroperitoneal Lymph Node Dissection

Two patients died postoperatively, for an operative mortality rate of 1.3 per cent. Both deaths occurred in patients with massive abdominal disease and were owing to hepatic failure, with the finding of progressive metastatic disease involving the porta hepatis, and not to any complication relating to the technical aspects of the operation. There were no deaths encountered in 109 patients with stages A and B disease. Over-all, there were 12 major (8 per cent) and 7 minor (4.7 per cent) complications among the 149 patients. Of the 12 major complications 9 occurred in 52 patients with stages B3 and C disease, while only 3 of 97 patients who underwent a thoracoabdominal approach for early stage disease sustained a major complication (3.1 per cent). The average hospital stay for the entire group of 149 patients was 11.2 days: 9.6 days for the 130 patients who did not have any complication, 13.9 days for the 7 with minor complications and 26.3 days for the 12 with major complications. These data indicate that thoracoabdominal radical retroperitoneal lymph node dissection is an extremely well tolerated procedure, with low morbidity and a high degree of effectiveness in controlling primary metastatic disease.
