It is referring to the Texan meaning of a ‘cowboy’, as someone who tries to do the right thing and passes on his knowledge, rather than to a fly-by night trader. Stephen Burkhart has been at the forefront of arthroscopic shoulder surgery for the past 20 years. With a background in mechanical engineering, he has been directly involved in describing and developing the biomechanical principles that underpin arthroscopic soft tissue repair. Two of his previous fellows are the co-authors and they have produced a book that not only comprehensively covers the most up to date techniques in arthroscopic shoulder surgery, both in text and DVD, but also brings together many of the senior authors’ ‘pearls of wisdom’.Although this book covers the basics of equipment, portal placement, knot tying and routine intra-articular and subacromial procedures, its real strength is in its description of the more advanced techniques. The well written and informative text is accompanied by good quality step-by-step arthroscopic photographs. There are also many well-drawn illustrations which are particularly useful in the chapter on ‘rotator cuff tear patterns: repairing a tear the way it ought to be’. The chapter on ‘understanding and recognising pathology; is over 50 pages long and comprehensively covers all the areas of biceps-cap-sulo-labral instability and rotator cuff pathology. The last third of the book is devoted to describing over 50 procedures using bullet-point text for the key steps with many arthroscopic photographs. There is a ‘tricks and tips’ section for each of the procedures.The accompanying DVD has over four hours of good quality arthroscopic footage and is narrated by the senior author. It is well edited and complements each of the technical chapters.This book is a tribute to its authors and is clearly the result of many painstaking hours of acquiring and editing images. It can be read from cover to cover but can also be used as a reference for particular procedures. However, a fair amount of arthroscopic experience and ability are required to fully appreciate, and gain the most from it.
