ResumoThe aim of this study was to describe fetal cardiac structure and function in pregnancies complicated by pre-eclampsia (PE) and/or fetal growth restriction (FGR) as compared to uncomplicated pregnancies. A prospective cohort study of: 112 pregnancies complicated by normotensive FGR (n=36), PE without FGR (n=34), PE with FGR (n=42) and 111 uncomplicated pregnancies, matched by gestational age at assessment. Fetal echocardiography was performed at diagnosis or recruitment for controls. B-type natriuretic peptide (BNP) was measured in cord blood collected at delivery. Cases complicated by PE and/or FGR showed similar signs of fetal cardiac remodelling (increased cardiothoracic ratio and myocardial wall thickness) and dysfunction (increased myocardial performance index and cord blood BNP). FGR groups also showed more spherical ventricles as compared to controls. Both PE and FGR are associated with fetal cardiovascular remodelling. Future research is needed to elucidate the mechanism(s) underlying these conditions.
