SUSTAIN: USERS’ EXPERIENCES WITH HOLISTIC PATIENT CARE AT HOME – A CASE-STUDY FROM NORWAY

ResumoIn Surnadal, a municipality in Mid-Norway, Holistic Patient Care at Home (HPH) is the framework for delivering health services to community-dwelling residents. Its goal is to seamlessly provide the right services at the right time and place. In collaboration with the European project SUSTAIN, Surnadal is promoting person-centeredness and care coordination in HPH. In 2017, some service users were interviewed to learn about their experiences with HPH. According to them, healthcare staff worked well together, were knowledgeable about their needs, and helped coordinate medication reviews with their physicians. The users also had the opportunity to provide some input regarding their care. However, they could have been more involved in goal setting and care planning. They also expressed that the staff did not engage them in comprehensive discussions about safety. During this session, we will discuss these areas for improvement, and HPH’s contribution towards person-centered and coordinated care in Surnadal.
