Resumo

Introduction: Pigmented Villonodular Synovitis (SVP) is a benign but aggressive lesion of the synovial membrane. Aim: To report a case of posterior SVP that was surgically corrected using an arthroscopic approach. Method: This is a single case study attended at the Orthopedics and Traumatology Service of Hospital Universitario Sao Francisco na Providencia de Deus - HUSF, located in the city of Braganca Paulista – SP, Brazil. Case Report: A 37-year-old male patient, whose preoperative resonance showed small to moderate joint effusion, associated with slight thickening of the synovial membranes, compatible with synovitis. An oval image of the posterior articular recess was observed, posterior to the anterior cruciate ligament, with well-defined limits, with a heterogeneous sign, intermediate in T1 and DP, in addition to a low signal area in its posterior portion, corresponding to a hematic component. All of these findings were absent on postoperative resonance. Conclusion: This study showed that surgical treatment of posterior villonodular synovitis is possible with an arthroscopic approach, even affecting the posterior knee region. In this sense, the technique presented is an interesting alternative available to orthopedic surgeons to treat patients with similar injuries.
