ResumoIntroduction: Arachnoid cyst associated with intracranial aneurysm is a rare condition. Its clinical presentation can lead to a wrong and late diagnosis, inappropriate conduct and clinical complications. The aim of this study is to review the literature cases of arachnoid cysts associated with aneurysm in order to elucidate the pathophysiology, the clinical presentations and the potential risk factors. Methods: PubMed, Bireme and UpToDate databases were from 1970 to February, 2014. Results: Twenty-nine patients with both abnormalities were found, fourteen female and fifteen male, the mean age was 47 years (range 6-71 years). Headache was the most frequent symptom. The majority of the cysts was in the temporal region (70%), and most of the aneurysms arouse from anterior circulation (92%), mainly in middle cerebral artery. These disorders can present in different forms: subarachnoid hemorrhage, subdural hemorrhage and intracystic hemorrhage, which is present in 71% of cases. Conclusions: The data found in this review highlight that intracystic hemorrhage can occur due to an aneurysm rupture and may leads to atypical symptoms.
