Análise comparativa da ressonância nuclear magnética com a artroscopia no diagnóstico das lesöes intra-articulares do joelho

ResumoOs autores analisaram os prontuarios de 37 pacientes submetidos a artroscopia do joelho e que haviam realizado ressonância nuclear magnetica previa ao ato cirurgico, com o objetivo de avaliar a eficacia deste metodo no diagnostico das lesoes do joelho. A sensibilidade, especificidade e acuracia do metodo foram, respectivamente, de 94%, 72% e 83% para menisco medial; de 81%, 80% e 83% para menisco lateral; de 53%, 95% e 78% para ligamento cruzado anterior; indefinida, 100% e 100% para ligamento cruzado posterior; e de 60%, 86% e 75% para condromalacia. Os exames foram realizados por apenas um radiologista e a artroscopia, por um unico cirurgiao, eliminando a variacao interpessoal. Concluiram que a ressonância nuclear magnetica e excelente exame para diagnosticar patologias meniscais, porem sua capacidade em verificar as lesoes do ligamento cruzado anterior e condromalacia nao corresponderam a expectativa. The purpose was analyzing and determining the efficacy of this method for knee lesion diagnosis. The sensitivity, specificity and accuracy of the method was respectively of 94%, 72% and 83% for medial meniscus; 81%, 80% and 83% for lateral meniscus; 53%, 95% and 78% for anterior cruciate ligament; undefined, 100% and 100% for posterior cruciate ligament; and 60%, 86% and 75% for chondromalacia. The exams were carried out by one radiologist only, and arthroscopy by one surgeon only, thus eliminating interpersonal variations. They have concluded that the magnetic resonance imaging is an excellent means of diagnosing meniscal pathologies; however, its capacity to verify lesions of anterior cruciate ligament and chondromalacia did not meet expectations.
