while supporting "spokes" of other disease-specific palliative clinic models.In co-located clinics, palliative providers work in close proximity to a referring disease specialty group while maintaining separate clinic space and independent scheduling templates.This clinic model can coordinate patient care between palliative and disease specialty groups while encouraging closer communication across a multidisciplinary team.Embedded palliative clinic models describe palliative providers who work within a disease specialty group and use common clinic space, staff, and scheduling templates with other subspecialty providers.This clinic model allows palliative providers to see patients in conjunction with their primary specialty team.In this model, disease specialists and palliative providers can share outpatient encounters for symptom assessment and goals-of-care discussions.In this session, we will explore benefits and challenges to different palliative clinic models and how these models have evolved to meet the need for outpatient palliative services across diverse medical systems nationwide.
