Successful outcome of Streptococcus pneumoniae-associated haemolytic-uraemic syndrome using washed blood products and exchange transfusion

ResumoA two-year-old boy with S. pneumoniae-associated haemolytic-uraemic syndrome (HUS) was treated with antibiotics and early peritoneal dialysis. The haemolysis and thrombocytopenia improved following a change in transfusion therapy to washed blood products and exchange transfusion. The exchange transfusion was performed after the red cell polyagglutination test had become negative and was associated with a dramatic improvement in haematocrit, platelet count and renal function. We suggest that transfusions of washed blood products (which do not provide anti-T-cryptantigen immunoglobulin) are more effective than standard blood products in this disease. In addition, the combination of early dialysis and appropriately timed exchange transfusion may have contributed to a nearly complete recovery from this potentially lethal form of HUS.
