Its cause and clinical significance is particularly obscure when it occurs in the absence of the nephrotic syndrome or a systemic disease such as diabetes mellitus or hypertension, impaired renal function, an abnormal urine sediment, anatomic alteration on excretory urography, or a past history of renal disease.
