Physical Clinical Care and Artificial-Intelligence-Guided Core Resistance Training Improve Functional Performance, Power, and Lumbar Isokinetics in Lower Back Pain Patients

Low back pain (LBP) is a common worldwide and is a substantial source of pain and disability. However, standardized exercise therapies for the treatment of LBP are limited in the literature. To evaluate the effectiveness of artificial intelligence (AI) guided core-focused resistance training on lumbar isokinetics, functional outcomes, and power of subjects with LBP. 108 adults were enrolled, 92 were included, and 69 subjects completed the 8-week intervention. Subjects were randomized into four groups (CTRL, TRAIN, CLIN, or COMBO). The CTRL group received no treatment. The TRAIN cohort received supervised AI-guided core-focused resistance training while the CLIN group received standard clinical care. The COMBO group received both clinical care and AI-guided training. Evaluation sessions were completed at baseline, 4 weeks, and 8 weeks to assess lumbar isokinetics, functional performance, and power. Baseline and 8 weeks outcomes were compared using matched pairs analysis. TRAIN, CLIN, and COMBO groups improved from baseline to 8 weeks for lumbar flexion torque at 60° and 90°, lumbar extension torque at 90°, deadlift strength, time to peak range of motion for overhead press, average power for deadlift, average power for bench, average power for lateral pulldown, and peak power during deadlift. Of note, COMBO subjects did not exhibit improved outcomes compared to CLIN or TRAIN alone. Treatment cohorts experienced greater improvements relative to the CTRL cohort. The improved biomechanics observed across all cohorts highlights the potential role of AI-guided core-focused resistance training in the treatment of chronic LBP alone or in coordination with clinical care.
