Fever of Unknown Origin and Hypoxemic Respiratory Failure Secondary to Histoplasma Capsulatum Infection in Systemic Sclerosis Related Interstitial Lung Disease: A Case Report

Resumo

We present a case of a male in his fifties with known systemic sclerosis-related interstitial lung disease and fever of unknown origin. His bronchoscopy specimen was reported as growing Blastomyces ; however, the final diagnosis on culture was Histoplasma . These organisms are both dimorphic fungi with a wide spectrum of pulmonary manifestations. On microscopy, it is possible to mistake one for the other based on the appearance of their early growth stages alone. Though severe infection from Histoplasma is relatively uncommon, both mycoses should be considered in patients with significant soil exposure and who reside in an endemic area.
