ResumoA 70-year-old female patient was referred for assessment of symptomatic buccal lesions that were present for 5 months. Extraoral examination revealed upper lip edema and lip commissure ulceration. Intraoral examination demonstrated oral ulcerations bearing a superficial granular mulberry-like aspect extending through the superior alveolar ridge, upper labial mucosa, buccal mucosa, and palate. Given the clinical presentation, the diagnostic hypothesis was paracoccidioidomycosis (PCM). Serologic investigation was ordered and an incisional biopsy was conducted. The microscopic examination confirmed the diagnosis of PCM, and serology was negative for Paracoccidioides brasiliensis ; however, the presence of Paracoccidioides lutzii was detected by Western blotting. The patient was referred to a pneumologist, who prescribed itraconazole (400 mg PO, O.D.). One month after treatment initiation, a complete resolution of the lesions was observed, and ongoing clinical monitoring has shown no signs of recurrence.
