ResumoThe current understanding of polycystic ovary syndrome (PCOS) diagnosis is contentious. The clinical findings are similar to those of many other endocrine and metabolic diseases. Key characteristics include hyperandrogenism and ovulatory dysfunction, which result in a slew of symptoms and signs that become the basis for clinical presentation. Adolescence presents a greater diagnostic challenge because of functional immaturity of reproductive hormonal physiology and various congenital steroidogenic and other metabolic defects. Similarly, the postmenopausal age group has significant differentials to address, such as androgen-producing ovarian and adrenal tumors. Such variations in clinical manifestations across the female reproductive age group, and in the postmenopausal phase, have made developing guidelines a difficult task over decades. This chapter discusses diagnostic criteria in adults, adolescents, and postmenopausal females. In addition, the differential diagnosis and referral process to specialists are described. It is also explained how a long-term follow-up should be planned and implemented in patients with PCOS.
