ResumoComparative studies to date between open and laparoscopic pyloromyotomy have been retrospective and report conflicting results. To scientifically compare the two techniques, the authors conducted the first large prospective, randomized trial between the two approaches. One mucosal perforation and one incisional hernia occurred in the open group. Late in the study, one patient in the laparoscopic group was converted to the open operation. Although Rammstedt's procedure has been performed as described for many years since its introduction, leading to pyloric stenosis being a completely survivable condition, focus has now shifted towards improving the quality of the surgical care provided. Adrian Bianchi described in 1986 the use of a circumumbilical incision to access pyloric stenosis, which provides the same visualization as a right-upper quadrant incision with almost imperceptible scarring.
