There is strong evidence that central visceral fat distribution in children and adolescents is associated with increased health risks. The use of body mass index to categorise childhood overweight and obesity requires clinical relevance of this proxy measurement of body fat. Measurement of waist as an indicator of increased visceral fat and an increased risk of particularly cardiovascular disease and type 2 diabetes has been used in adults for some time. Although rare, monogenetic causes of obesity in children and young people will sometimes be seen in a childhood weight management service. The most common inherited disorders seen in routine childhood weight management clinical practice will be: Down's syndrome, Prader-Willi syndrome, Duchenne muscular dystrophy, and Fragile.
