The year 2024 will be Annals of Neurology's busiest year ever, with over 2,202 manuscripts already received as of December 1, 2024, including 1,906 original reports and brief communications. This represents a remarkable +22% increase in total manuscripts and a 25% increase in original reports compared to last year at the same point. We are an increasingly international journal. In 2024, nearly 80% of our submissions came from outside the United States and Canada. Authors including those from Asia now account for over 45% of submissions, those from Europe and the United Kingdom for about 25%, and those from North America about 20%. Acceptance rates vary significantly for articles from these different regions. For example, articles from the United States have an overall acceptance rate of ~25%, those from the United Kingdom and Europe ~15%, and those from Asia ~5%. We try to handle submitted manuscripts expeditiously. Over 80% of the manuscripts submitted receive an initial decision within 30 days, and over 98% within 60 days. We remain highly selective. Only 29% of our manuscripts are sent for external review. Of this group, ~30% to 40% will ultimately be accepted. In aggregate, this puts our acceptance rate for manuscripts submitted at about 8% to 12%. We continue to offer a special benefit to Fellows of the American Neurological Association (ANA) and Child Neurology Society (CNS) who are first, senior, or corresponding authors of submitted Research Articles or Brief Communications, enabling them to designate one original research article per calendar year for guaranteed external review. Annals has an extraordinarily close relationship with our sister journal Annals of Clinical and Translational Neurology (ACTN). ACTN's Editor-in-Chief, Dr Ahmet Hoke, joins our weekly editorial discussion meetings. We offer authors a transfer option to ACTN for a subset of articles rejected by Annals without external review (~444 year to date [YTD]) and another subset rejected after external review (~131 YTD). About 40% of authors accept this option, and we hope to see this rate continue to rise. These articles are typically of high quality, and over 80% of those transferred after external review by Annals, are ultimately accepted by ACTN. A similar transfer option was also recently initiated with the Annals of the Child Neurology Society (ACNS), a new official journal of the CNS, with Dr E Steve Roach as its inaugural Editor-in-Chief Despite being highly selective with manuscripts we decide to send out for external review, this still results in over 700 articles per year needing reviewers. We try to obtain at least 2 reviewers per manuscript. The quest for reviewers gets more challenging every year. Our overall acceptance rate for review invitations is around 28%—meaning that we need to send about 5,000 review invitations annually to secure at least 2 reviews per article. We are exploring creative ways to increase this reviewer invitation acceptance rate, including supporting peer review training courses and potentially offering guaranteed review or other journal submission benefits for frequent reviewers. Serving as a reviewer for articles submitted to medical and scientific journals, including Annals, remains both uncompensated and under-recognized. Our reviewers can have their review activities documented in Clarivate's Web of Science (previously Publons), and our publisher Wiley sends reviewers Certificates of Appreciation. A list of our nearly 1,000 reviewers in 2024 is attached at the end of this report, with those performing 3 or more reviews noted with an asterisk. We are extremely grateful to all these individuals for their service, and for their key role in influencing the quality of the articles we ultimately publish. Editorial Board members serve a 3-year term, with an option for renewal, and about one third of the Editorial Board turns over annually. New Editorial Board members are nominated by our Associate and Assistant Editors. These nominees are typically among our most frequent reviewers and are selected based both on their expertise and on the frequency and quality of the reviews they provide. We want to extend our sincere thanks to those completing this service, and a warm welcome to our new Editorial Board Members. Both groups are listed at the end of this report. Annals is the official journal of both the ANA and the CNS, and the manuscripts we receive include virtually all aspects of both adult and pediatric neurology. In 2024, manuscripts in vascular neurology, movement disorders, and dementia/behavioral neurology each accounted for ~20% of our submissions, with those in epilepsy, neuromuscular, and neuroimmunology each adding an additional ~10%. An informal study of the ~175 original articles published in 2024 (volumes 95 and 96) suggests a close parallel between the subject distribution of manuscripts received and the distribution of our published content. Movement disorders and dementia/behavioral neurology each accounted for ~19% of content; vascular neurology and neuroimmunology were at about 15%; epilepsy at 13%; and neuromuscular at about 7% (with other areas accounting for the remainder). Currently about two-thirds of our original research articles and brief communications are "open access". We set a goal of increasing the number of reviews in Annals without limiting original contributions. Annals has historically lagged behind its peer journals in the percentage of its content devoted to reviews. Thanks in large part to the efforts of our Review Editor, Dr Eva Feldman, with the assistance of Dr Stacey Jacoby, we are well on our way to achieving this goal. We published 16 reviews in 2024, with 10 of our 12 monthly issues having at least 1 review and 6 issues having 2 reviews. Many reviews are now accompanied by graphical abstracts. Reviews published in 2024 included a diverse set of fields and topics among them being amyotrophic lateral sclerosis (ALS), multiple sclerosis (MS), Alzheimer's Disease, Vascular Neurology, Biomarkers in Neurological Disease, CAR T-cell Therapies in Immune-mediated Neurological Disorders, Exosomes in Neurodegenerative Diseases, and Gender Diverse Research in Neurology. All of this content was available as open access. Our "Neurogenesis" section, the literal brainchild of my predecessor Dr Clifford Saper, included articles related to academic neurology throughout the career span. These included a guide for aspiring neurology educators, a discussion of the burdens of regulatory compliance on physician-scientists, and a summary of the success achieved by the NINDS R25 program. We also published a piece from the Association of University Professors of Neurology (AUPN) on fostering faculty development for women in neurology. Challenges always exist for scientific journals, such as Annals. Two that are worth noting deal with preventing publication of articles with manipulated or fraudulent data, and how journals will deal with the increasingly stringent federal policies related to timely open access to federally funded research. Misconduct related to scientific publications has been an unfortunate staple in the news in 2024. Annals routinely scans all articles being considered for acceptance with plagiarism detection software (iThenticate). Several commercial software programs are now available that utilize artificial intelligence (AI)-based software to evaluate the integrity, and detect manipulation of, figures including Western blots and microscopy images. In 2025, Annals, with the assistance of staff at our publisher, will also begin checking articles being considered for acceptance for inappropriate image manipulation. By 2026, all federally funded research results will need to be open access immediately. Agencies are still updating their specific open access and data sharing plans and guidelines to implement this directive. About 40% of the original research articles published by Annals indicate that aspects of their studies were supported by National Institutes of Health (NIH) grants. As noted about two-thirds of Annals content is already open access, and we will continue to work closely with our publisher to insure we adapt to and comply with the relevant federal policies. These are the individuals that make sure that all the critical day-to-day aspects of dealing with all those incoming manuscripts, communicating with authors and reviewers, and assembling the journal, actually get done.
